Esta determinaci\u00f3n, salvo en valores extremos (&lt; 90% o 100%), se correlaciona con aceptable <strong>fiabilidad con la presi\u00f3n arterial de ox\u00edgeno<\/strong>, lo que permite conocer, de forma continua y segura, el aporte de ox\u00edgeno a los tejidos, <strong>dosificar la administraci\u00f3n de oxigenoterapia <\/strong>y <strong>valorar <\/strong>su respuesta en el <strong>paciente<\/strong>.<\/p>\n\n\n\n<p>Se fundamenta en la capacidad de la hemoglobina para absorber luz a diferente longitud de onda, seg\u00fan est\u00e9 reducida o saturada de ox\u00edgeno. <strong>Requiere un sensor cut\u00e1neo, con un emisor de luz y un receptor contrapuesto, para cuantificar la absorci\u00f3n lum\u00ednica por los tejidos y su cambio tras cada latido arterial, calculando as\u00ed el porcentaje de oxihemoglobina.<\/strong><\/p>\n\n\n\n<p>El sensor debe colocarse en una<strong> zona que permita el paso del haz lum\u00ednico y que contenga un pulso arterial adecuado<\/strong> para que la determinaci\u00f3n sea correcta: lecho ungueal en manos o pies, l\u00f3bulo auricular, borde cubital de la mano y borde externo del pie en los neonatos. <strong>Se obtiene as\u00ed un valor preciso de SatO2, onda de pulso arterial y frecuencia cardiaca.<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter size-large is-resized\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/staging.campusvygon.com\/es\/wp-content\/uploads\/sites\/2\/2022\/06\/AdobeStock_419879095-1024x540.jpeg\" alt=\"\" class=\"wp-image-34283\" width=\"768\" height=\"405\" \/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Limitaciones de la pulsioximetr\u00eda a tener en cuenta<\/h3>\n\n\n\n<p>Hay situaciones cl\u00ednicas y\/o ambientales en las que debemos interpretar la SatO2 con m\u00e1s cuidado por posibilidad de error en la determinaci\u00f3n:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Inadecuada captaci\u00f3n de pulso arterial.<\/strong> En este caso, la determinaci\u00f3n no es v\u00e1lida.<\/li><li>Fuente de <strong>luz <\/strong>ambiental <strong>intensa <\/strong>(en general producen descenso de la cifra de SatO2).<\/li><li>Posible <strong>interferencia <\/strong>con otros aparatos electr\u00f3nicos.<\/li><li>Mantenimiento deficiente del equipo, sensor inadecuado al paciente (tama\u00f1o, fijaci\u00f3n, etc.).<\/li><li><strong>Elementos que dificulten la absorci\u00f3n de la luz:<\/strong> esmalte de u\u00f1as, tatuajes, etc. La ictericia no produce interferencias con la determinaci\u00f3n de SatO2.<\/li><li>Contrastes intravenosos con absorci\u00f3n lum\u00ednica similar a Hb.<\/li><li>Los movimientos del sensor producen artefactos y <strong>distorsi\u00f3n <\/strong>de la medida, restando fiabilidad.<\/li><li><strong>Mala perfusi\u00f3n perif\u00e9rica<\/strong> con pulso d\u00e9bil como causa m\u00e1s frecuente de lectura err\u00f3nea: hipotensi\u00f3n, hipovolemia, hipotermia, baja temperatura ambiental, etc.<\/li><li><strong>Aumento de pulso venoso<\/strong> (fallo cardiaco derecho) puede distorsionar la cifra deSatO2.<\/li><li><strong>Anemias graves <\/strong>pueden dar lecturas falsas (Hb &lt; 5%). Valores normales de SatO2 no garantizan un aporte adecuado de ox\u00edgeno a los tejidos.<\/li><li>Algunas <strong>dishemoglobinemias <\/strong>(carboxihemoglobina y metahemoglobinemia) producen lecturas falsamente normales por su absorci\u00f3n de luz similar a la oxihemoglobina. Cuidado en las posibles intoxicaciones por CO.<\/li><li><strong>Anemia falciforme<\/strong> en la que se puede sobreestimar la determinaci\u00f3n (hasta el 7%) en casos de hem\u00f3lisis con aumento significativo de la carboxihemoglobina; o subestimar los valores de SatO2 por las caracter\u00edsticas de la hemoglobina S (desplazamiento de la curva de disociaci\u00f3n de Hb a la derecha) con valores bajos de SatO2 y valores normales o altos de presi\u00f3n arterial de ox\u00edgeno.<\/li><li><strong>Valores extremos de SatO2 <\/strong>(&lt; 80% o 100%) no se correlaci\u00f3n con cifras reales de PaO2.<\/li><li>Recordar que durante la noche se producen <strong>descensos puntuales<\/strong> y transitorios de SatO2 en todos los individuos sanos, con independencia de la edad.<\/li><li>No aporta otros datos importantes en la valoraci\u00f3n completa del enfermo respiratorio (PaO2, PaCO2, pH, bicarbonato). No equivale ni sustituye a la gasometr\u00eda arterial.&nbsp;<\/li><li>La pulsioximetr\u00eda, a pesar de sus limitaciones, es el elemento b\u00e1sico de control en oxigenoterapia en la edad pedi\u00e1trica (amplia distribuci\u00f3n, bajo coste, simplicidad de uso y escaso mantenimiento) y, salvo en pacientes seleccionados, no se precisa de otros elementos de monitorizaci\u00f3n.<\/li><li>Se realizar\u00e1 gasometr\u00eda y\/o capnograf\u00eda en pacientes con insuficiencia respiratoria grave (posible atenci\u00f3n en UCI) y pacientes con riesgo de hipoventilaci\u00f3n (enfermos con patolog\u00eda neuromuscular, oxigenoterapia cr\u00f3nica, etc.).<\/li><\/ul>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">El papel de la pulsioximetr\u00eda en los dispositivos de oxigenaci\u00f3n autom\u00e1ticos<\/h3>\n\n\n\n<p>El ox\u00edmetro permite monitorizar y registrar los par\u00e1metros cardiorrespiratorios convencionales y, tras el an\u00e1lisis de la se\u00f1al pletismogr\u00e1fica, extraer otros par\u00e1metros fisiol\u00f3gicos como la frecuencia respiratoria.
