un elemento que debemos tener muy en cuenta a la hora de valorar un <strong>Dispositivo de Acceso Vascular<\/strong> (DAV).<\/p>\n<h2><strong><u>Clasificaci\u00f3n de los materiales<\/u><\/strong><\/h2>\n<p>Dentro de los DAV de <strong>media larga duraci\u00f3n<\/strong>, familia a la que pertenecen los <strong>reservorios<\/strong>, se distingue dos materiales: el <strong>Poliuretano<\/strong> (PUR) y la <strong>Silicona<\/strong> (Si).<\/p>\n<p>El PUR se puede dividir en dos categor\u00edas:<\/p>\n<ul>\n<li>Para media duraci\u00f3n es decir <strong>&lt;30 d\u00edas<\/strong>: PUR clase <strong>IIa<\/strong><\/li>\n<li>Para larga duraci\u00f3n es decir <strong>&gt;30 d\u00edas<\/strong>: PUR de <strong>clase III<\/strong>.<\/li>\n<\/ul>\n<p>La silicona entra en la <strong>clase<\/strong> <strong>III <\/strong>dentro de la clasificaci\u00f3n de producto, por lo que es considerada un material para acceso vascular de larga duraci\u00f3n.<\/p>\n<p>Siguiendo esta clasificaci\u00f3n, vemos que el <strong>reservorio<\/strong>, dentro de los DAV de media-larga duraci\u00f3n, queda ubicado de la manera siguiente:<a href=\"https:\/\/staging.campusvygon.com\/es\/material-los-reservorios-poliuretano-silicona\/tabla-material-kt\/\" rel=\"attachment wp-att-14893\"><img loading=\"lazy\" decoding=\"async\" class=\" cuenta que aqu\u00ed no se incluye el acceso vascular neonatal (el PICC neonatal representa un caso particular: puede ser de silicona, es entonces de clase III pero es considerado de media duraci\u00f3n).<\/em><\/p>\n<h2><\/h2>\n<h2><strong><u>Diferencia entre PUR y Silicona en cat\u00e9teres de reservorios<\/u><\/strong><\/h2>\n<p>Es importante conocer las caracter\u00edsticas de estos dos materiales a la hora de <strong>elegir de forma adecuada el reservorio<\/strong>:<a href=\"https:\/\/staging.campusvygon.com\/es\/material-los-reservorios-poliuretano-silicona\/tabla-pur-si-reservorios\/\" rel=\"attachment wp-att-14894\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-14894 aligncenter\" src=\"https:\/\/staging.campusvygon.com\/es\/wp-content\/uploads\/sites\/2\/2019\/12\/tabla-pur-si-reservorios.png\" alt=\"\" width=\"709\" height=\"709\" de cada material son particularmente interesantes si consideramos:<\/p>\n<ol>\n<li>la <strong>zona de inserci\u00f3n<\/strong> elegida<\/li>\n<li>el <strong>flujo<\/strong><\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<ol>\n<li>Cada material se va a comportar de una manera cuando la vena elegida <strong>no supone un recorrido lineal <\/strong>del cat\u00e9ter: en el acceso <strong>yugular<\/strong>, la tunelizaci\u00f3n puede implicar una <strong>curva cerrada <\/strong>del cat\u00e9ter cuando sale de la vena y entra a nivel subcut\u00e1neo<\/li>\n<\/ol>\n<p><a href=\"https:\/\/staging.campusvygon.com\/es\/material-los-reservorios-poliuretano-silicona\/yugular-dibujo-2\/\" rel=\"attachment wp-att-14895\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-14895 aligncenter\" src=\"https:\/\/staging.campusvygon.com\/es\/wp-content\/uploads\/sites\/2\/2019\/12\/yugular-dibujo-2.jpg\" alt=\"\" \/><\/a><\/p>\n<p><strong>\u00a0<\/strong>En este caso el <strong>PUR es m\u00e1s propenso a acodarse por ser m\u00e1s r\u00edgido<\/strong>. Este fen\u00f3meno puede tener m\u00e1s importancia en ni\u00f1os peque\u00f1os ya que el margen de maniobra en cuanto al recorrido del cat\u00e9ter es reducido.<\/p>\n<p>La <strong>silicona es m\u00e1s indicada<\/strong> ya que <strong>su flexibilidad evita los acodamientos<\/strong> en todo el recorrido a pesar del \u00e1ngulo que pueda tener el cat\u00e9ter. Aun as\u00ed, es importante recordar que las recomendaciones indican que para evitar al m\u00e1ximo el fen\u00f3meno de acodamiento en el acceso yugular, la <strong>punci\u00f3n se tiene que hacer en el tramo m\u00e1s inferior posible de la vena yugular<\/strong>.<\/p>\n<p>&nbsp;<\/p>\n<p>Como hemos visto anteriormente la <strong>silicona<\/strong> es un material <strong>m\u00e1s fr\u00e1gil<\/strong> y por esta raz\u00f3n los cat\u00e9teres de silicona tienen una <strong>pared m\u00e1s gruesa<\/strong> para poder resistir mejor a la tracci\u00f3n.
