Carlos Salazar sobre control del dolor postoperatorio en cirug\u00eda de mama"},"content":{"rendered":"<p>El c\u00e1ncer de mama es el c\u00e1ncer m\u00e1s frecuente en las mujeres en Espa\u00f1a. De hecho, esta patolog\u00eda supone un riesgo para 1 de cada 8 mujeres en Espa\u00f1a. El tratamiento de la enfermedad consta de: cirug\u00eda, quimoterapia, radioterapia, hormonoterapia e inmunoterapia. El dr. Carlos Salazar, adjunto de anestesia en el hospital 12 de Octubre comparte hoy con nosotros sus conocimientos sobre una de estas ramas del tratamiento del c\u00e1ncer de mama: la cirug\u00eda.<\/p>\n<p>En concreto, el dr. Salazar nos explica c\u00f3mo podemos controlar el dolor postoperatorio de la cirug\u00eda de mama para evitar que se cr\u00f3nifique y nos presenta el protocolo ERAS que se puso en marcha en el hospital de Figueras, donde trabajaba anteriormente, y que est\u00e1 a punto de ponerse en marcha en el 12 de Octubre por sus buenos resultados.<\/p>\n<p><iframe loading=\"lazy\" title=\"Entrevista: Dolor postoperatorio en cirug\u00eda de mama - Dr. Carlos Salazar\" width=\"1080\" height=\"608\" src=\"https:\/\/www.youtube.com\/embed\/z2U4LEqpILk?feature=oembed\"  allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture\" allowfullscreen><\/iframe><\/p>\n<blockquote><p>El dr. Salazar explica que, con este protocolo se utilizan m\u00e9todos de anestesia combinada, en la que se utilizan bloqueos espec\u00edficos como el paravertebral y una sedaci\u00f3n profunda o una anestesia general. As\u00ed, Salazar afirma que \u00abel dolor por la lesi\u00f3n de la cirug\u00eda tiene un stop en la puerta de entrada al sistema nervioso porque nos nervios que est\u00e1n en la mama no trasladan el dolor a la m\u00e9dula y esta no lo transmite al cerebro\u00bb.<\/p><\/blockquote>\n<p>En el postoperatorio se combina paracetamol con un antiinflamatorio y un cat\u00e9ter que funciona como una bomba paravertebral, que le permite auto-dosificarse bolos extra para limitar el dolor. As\u00ed el doctor se\u00f1ala que menos del 10% de las pacientes necesitaba analgesia de rescate y obten\u00edan una media de 1,8 en el \u00edndice EVA.<\/p>\n<p>Por este motivo, la mayor\u00eda de las pacientes recib\u00edan el alta en 48 o 72 horas, lo cual supone una mejora evidente al tiempo medio de estancia en el hospital tras una intervenci\u00f3n de este tipo. Adem\u00e1s, la eliminaci\u00f3n del dolor garantiza una recuperaci\u00f3n m\u00e1s r\u00e1pida.<\/p>\n<p>Por \u00faltimo, el dolor cr\u00f3nico postmastectom\u00eda, que est\u00e1 en una media del 30% de los casos, se consigui\u00f3 reducir en el hospital hasta un 4%. Todos estos resultados demuestran que un mayor peso de la anestesia regional, combinada con t\u00e9cnicas de anestesia general, as\u00ed como el uso de bloqueos analg\u00e9sicos continuos en el postoperatorio, pueden mejorar mucho los resultados para las pacientes y provocar un uso de recursos sanitarios mucho m\u00e1s eficiente.<\/p>\n<h3>Qu\u00e9 tipo de bloqueo es m\u00e1s efectivo para controlar el dolor postoperatorio en cirug\u00eda de mama<\/h3>\n<p>Tras esta explicaci\u00f3n sobre el protocolo ERAS llevado a cabo en el hospital de Figueras, el dr. Salazar hace un repaso t\u00e9cnico a las diferentes t\u00e9cnicas que nos pueden ayudar a garantizar el bienestar y la pronta recuperaci\u00f3n del paciente, as\u00ed como un alta temprana.<\/p>\n<p><iframe loading=\"lazy\" title=\"Webinar - Dolor postoperatorio en cirug\u00eda de mama  protocolo ERAS y t\u00e9cnicas de infiltraci\u00f3n\" width=\"1080\" height=\"608\" src=\"https:\/\/www.youtube.com\/embed\/HXqWFmKZ6Xs?feature=oembed\"  allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture\" allowfullscreen><\/iframe><\/p>\n<p>Salazar se\u00f1ala que para realizar estas t\u00e9cnicas de anestesia regional, es muy importante tener amplios conocimientos de anatom\u00eda, sin embargo, considera que la anestesia regional es muy beneficiosa en estos casos ya que:<\/p>\n<ul>\n<li>Reduce el dolor agudo<\/li>\n<li>Reduce el dolor cr\u00f3nico<\/li>\n<li>Reduce el consumo de opiaceos<\/li>\n<li>Reduce las nauseas<\/li>\n<li>Optimiza la ambulatorizaci\u00f3n de las cirug\u00edas<\/li>\n<li>Mejora la satisfacci\u00f3n del paciente<\/li>\n<li>Permite la cirug\u00eda despierta en algunos casos muy concretos<\/li>\n<li>CONTROVERSIA: algunos estudios muestran que la anestesia regional dificulta que el dolor severo llegue a la m\u00e9dula y, por tanto, limita la recurrencia del mismo. El dr. Salazar insiste en que hacen falta m\u00e1s estudios que demuestren este \u00faltimo punto y que, por el momento, se trata de un debate muy controvertido.<\/li>\n<\/ul>\n<p>Entre los diferentes bloqueos disponibles para llevar a cabo esta t\u00e9cnica, el dr.
